The Nigerian Safety Investigation Bureau (NSIB) has launched an investigation into a serious aviation incident involving a Fly Bird HS 125 aircraft, registration number 5NKAL.

The aircraft, which departed Nnamdi Azikiwe International Airport, Abuja, on December 6, 2024, for Kotoka International Airport, Accra, experienced multiple engine failures during its flight.

According to preliminary reports, the aircraft, carrying three crew members and one passenger, encountered technical difficulties while climbing through flight level 240, en route to flight level 280.

At approximately 17:54 UTC, the crew reported an issue with engine No. 2, prompting a request to return to Abuja.

The request was promptly granted, and the aircraft began descending to flight level 190.
During the descent, the situation worsened as the aircraft lost engine No. 2 at flight level 230.

The crew continued their return to Abuja but declared a “Mayday” (distress signal) near the airport at 5,000 feet when engine No. 1 also failed.

Despite these critical challenges, the crew successfully executed an emergency landing at Abuja Airport at 18:16 UTC. All four individuals on board were unharmed.

Speaking on the incident, NSIB Director of Public Affairs, Mrs. Bimbo Olawumi Oladeji, emphasized the Bureau’s commitment to uncovering the root cause of the engine failures.

“Our Go Team is currently collecting evidence, conducting interviews, and analyzing flight data and maintenance records to determine the circumstances that led to this serious incident. The primary goal is to prevent future occurrences and enhance aviation safety,” Mrs. Oladeji said.

She added that updates would be provided as the investigation progresses, ensuring transparency and accountability in the process.

The statement added that NSIB’s swift response highlighted its dedication to promoting safety in Nigeria’s transport sector.

“As a multimodal transport investigation agency, the NSIB focuses on identifying probable causes of transport incidents and recommending actionable measures to prevent recurrences.

“For inquiries or updates, the NSIB can be reached at [email protected] or via its social media channels (@NSIB_Nigeria). An emergency contact line is also available at +234-807-709-0909”, the statement added.
